Alisha's Happy Paws LLC Contract

For the purposes of this document, the terms Client, Pet Owner are synonymous with the person contracting services for one or more domestic animals. Alisha's Happy Paws, LLC, Alisha's Happy Paws, Alisha's Happy Paws Pet Sitters and walkers are synonymous with the pet care company providing care for the Client below.

Alisha's Happy Paws, LLC provides services to the Pet Owner/Client below which includes, but is not limited to: dog walking, pet sitting, vacation sitting, transporting, feeding, and caring for pets.

This agreement is entered into by and between Alisha's Happy Paws, LLC and the undersigned Client of Alisha's Happy Paws, LLC and is effective immediately, upon acknowledgement of both parties.

Client desires to engage Alisha's Happy Paws, LLC and its contracted pet sitters and dog walkers to provide pet care and agrees to the terms below:

Injury to Client animal(s) by another animal:
Alisha's Happy Paws, LLC and its contracted staff will not be responsible for any animal(s) that instigate fights with other animals or is injured by another animal while in Alisha's Happy Paws, LLC care. Alisha's Happy Paws, LLC will take all appropriate actions to ensure that Client's pet(s) are not placed in the company of aggressive or violent animals and/or will immediately remove Client's animal(s) from any animal fight.

Injury or escape of animals:
Client and sitter are aware that the highest level of care shall be given to any and all pets under care of the pet sitter employed by Alisha's Happy Paws, LLC. Client is aware that pet(s) are instinctual in nature and not always controllable by the sitter even when the highest level of care is provided. Should the Client's animal(s) run away or be injured, the Client agrees that Alisha's Happy Paws, LLC will not be responsible for any liability. The above waiver of liability in favor of Alisha's Happy Paws, LLC shall not apply or be effective if Alisha's Happy Paws, LLC conduct in providing its pet care services is found to be grossly negligent, reckless, or if there is intentional misconduct.

Injury to Pet Sitter or another person by Client's Animal(s):
Client will be responsible for all medical expenses and damages resulting from an injury to pet sitter or other persons by the Client's pet(s). Client agrees to pay the full cost of any and all medical expenses and damages should they arise to the Alisha's Happy Paws, LLC pet sitter or the pet sitter's possessions as a result of the Client's animal(s) actions. Client agrees to indemnify, hold harmless, and defend Alisha's Happy Paws, LLC in the event of a claim by any person injured by the Client's pet(s).

Unsafe Pet(s): This contract permits Alisha's Happy Paws, LLC to use discretion to stop and end service at any time that Client's pet(s) pose a danger to the safety and health of itself, other pets, other people, or Alisha's Happy Paws, LLC sitters/walkers.

Off-leash Dog(s): Some of our Clients request off-leash activity and play for their dog(s). Client is aware and agrees that the Client's dog(s) may be off-leash in designated dog parks and other legal-for-dogs areas in order to provide maximum exercise and play for Client's dog(s). Client agrees that should the Client's dog(s) run away or be injured during the off-leash hike and play time, neither Alisha's Happy Paws, LLC nor Alisha's Happy Paws, LLC walkers/sitters will not be held liable for any damages resulting to people or other animals or to the Client's dog(s).

If Client has a fully fenced in yard or fully fenced in and working electric fence it is the Client's responsibility to keep any confident for the pets in working condition and to alert Alisha's Happy Paws, LLC if any confident is faulty in a timely manner prior to Client's next visit. Alisha's Happy Paws, LLC never recommends leaving a pet outside unattended. If Client has doggy doors, Alisha's Happy Paws, LLC never recommends having them open when no one is home with the pet. If this is the preference of the Client, the Client understands the risks and will not hold Alisha's Happy Paws, LLC or Alisha's Happy Paws, LLC walkers/sitters liable for any damages, injuries, or death resulting from pet being unattended outside including and not limited to people, other animals, the Client's pet(s), outside damage, missing pet, ect.

Alisha's Happy Paws, LLC will use the highest level of care to any and all pet(s). Client is aware that pet(s) are instinctual in nature and not always controllable by the sitter even when the highest level of care is provided. Should the client's pet(s) run away or be injured while sitter is entering or exiting the home or during any point of the visit the client agrees that Alisha's Happy Paws, LLC will not be held responsible for any liability.

Solicitation of Alisha's Happy Paws, LLC Sitters: Client agrees that the pet sitter provided by Alisha's Happy Paws, LLC is employed through Alisha's Happy Paws, LLC and is contracted to work only through Alisha's Happy Paws, LLC and not directly through the individual Client. Client agrees that all reservations for present and future sitting and dog walking must be made through Alisha's Happy Paws, LLC and not the individual sitter. Client is aware that the Alisha's Happy Paws, LLC sitter may give out his/her personal number to the Client. Client agrees to use this number only in the event of an emergency during which the sitter/walker is already employed directly through Alisha's Happy Paws, LLC for Client. Client agrees that this number shall not be used for any solicitation of future pet sitting or dog walking. Client is aware that should this contract be broken, legal fees, and misuse-of-staff charges will apply.

Client Home Care Needs: Alisha's Happy Paws, LLC is not responsible for wilted, dead or otherwise unhealthy plants. Alisha's Happy Paws, LLC sitters will work hard to follow written instructions as precisely as possible, but cannot be responsible if the results are not favorable to the Client. Please place all indoor plants together on a waterproof surface in plain sight as your pet sitter is not responsible for water-damaged areas or missed plants.

Alisha's Happy Paws, LLC is not responsible for damage to the home beyond the control of the sitter. This includes, but is not limited to: electrical problems, leaks, and acts of nature. All repairs to home and related fees will be paid by Client or fully reimbursed to Alisha's Happy Paws, LLC within seven (7) days of Client's last visit unless otherwise discussed.

Alisha's Happy Paws, LLC is not responsible for any damage to the property or home of the Client unless such damage is caused by the negligent act of Alisha's Happy Paws, LLC providers.

Alisha's Happy Paws, LLC is not liable for any loss or damage in the event of a burglary or other crime that should occur while under this contract. Client agrees to secure home prior to leaving the premises. Alisha's Happy Paws, LLC will attempt to re-secure the home according to Client instructions at the end of each visit.

Client is responsible for pet-proofing house and yard and security fences/gates/latches. Alisha's Happy Paws, LLC will not be responsible for the safety of the pet(s) and will not be liable for the death, injury, disappearance or legal consequences of any pet(s) with unsupervised access.

If Alisha's Happy Paws LLC or its providers are asked by Client to return to Client's home during a non scheduled visit due to and not limited to electrical problems, leaks, and acts of nature to allow someone Client has set up to fix there is a fee of our $58 minimum and/or $58 per hour if more than an hour is required. Client is responsible for all repairs to home and related fees will be paid by Client or fully reimbursed to Alisha's Happy Paws, LLC.

Terms:

The terms of this contract apply to all pets owned by the Client, including any and all new pets that the Client obtains on or after the date that this contract is acknowledged,at any and all locations the Client designates for service.

By acknowledging in the app Precise Petcare to the terms of Alisha's Happy Paws, LLC, the Client agrees they have read this agreement in its entirety and fully understands and accepts its terms and conditions. To request any and all visits with Alisha's Happy Paws I, the Client agree to these terms and fully understand this agreement entirely.

I have read the above and by acknowledging in the app Precise Petcare, I agree to the conditions listed above.

I the Client, understand the above contract and I am in acknowledgement that I am hereby legally bound.

I am the SOLE owner of the pet(s) in care of Alisha's Happy Paws, LLC or have shared this contract with ALL owner(s) of the pet(s). ALL possible owners of the pet(s) have agreed to the above contract and are legally bound.

Alisha's Happy Paws LLC- Payment Policies, Cancellation Policies, Additional Fees, & Policies
Payment Policy / Cancellation Policy:

Dog Walking, Reoccurring Visits, Non-Vacation Pet Sitting:
All payment for dog walking and non-vacation pet sitting is due no later than 48 hours before start of visit(s) unless other options have been discussed. A credit card is required to hold a reservation. Accepted payment methods are credit card through the Precise Petcare App. Client gives Alisha's Happy Paws, LLC authorization to automatically bill the card on file through the Precise Petcare App. Should a transaction be declined for any reason Client agrees to provide a different credit card to Alisha's Happy Paws, LLC before the next visit can take place.

Client agrees to the Alisha's Happy Paws, LLC cancellation policy which is as follows:
Alisha's Happy Paws, LLC Dog Walking, Reoccurring Visits, Non-Vacation visits cancellation policy is as follows:
A Full 48 hour notice for cancellation prior to first visit: FULL REFUND
Less than 48 hour notice: NO REFUND OR CREDIT GIVEN TO CLIENT FOR VISIT THAT IS LESS THAN 48 HOURS NOTICE. If more than one visit will need cancellation the visits given more than 48 hour notice will be fully refunded to the client.

Reoccurring visits and/or Regular monthly dog walk Clients agree to give two (2) weeks' notice should Client decide to discontinue regular ongoing dog walking service or visits. If Client cannot or does not give two (2) weeks' notice, Client agrees to compensate Alisha's Happy Paws, LLC for what would have been the total for two weeks of regular dog walking service. Alisha's Happy Paws, LLC agrees that should Client discontinue service due to blatantly poor care/service from walker for any reason, then this two (2) week notice addendum will not be enforced by Alisha's Happy Paws, LLC.

Last Minute Fee (LMF) for Non-Vacation Visits
Client agrees to pay an additional $10 per visit last-minute fee (LMF) to Alisha's Happy Paws, LLC for any visit or dog walk that is requested by Client less than 48 hours out from start time of reservation

Payment Policy / Cancellation Policy:
Vacation Dog Walking & Vacation Pet Sitting:

All payment for vacation pet sitting is due 7 days before Client's start of visit(s) unless other options have been discussed. A credit card is required to hold a reservation. Accepted payment methods are credit card through the Precise Petcare App. Client gives Alisha's Happy Paws, LLC authorization to automatically bill the card on file through the Precise Petcare App. Should a transaction be declined for any reason Client agrees to provide a different credit card to Alisha's Happy Paws, LLC before the next visit can take place.

Client agrees to the Alisha's Happy Paws, LLC cancellation policy which is as follows:
Alisha's Happy Paws, LLC Vacation Visits Cancellation Policy is as follows:
8 or more days' notice for cancellation prior to reservation: FULL REFUND GIVEN TO CLIENT.
4-7 days' notice for cancellation: 50% OF TOTAL FEE REFUNDED TO CLIENT, NO CREDIT.
3 or fewer days' notice for cancellation: NO REFUND OR CREDIT GIVEN TO CLIENT; FULL RESERVATION FEE WILL BE CHARGED TO CLIENT.

Last Minute Fee (LMF) for Vacation Visits
Client agrees to pay an additional $10 per visit last-minute fee (LMF) to Alisha's Happy Paws, LLC for any reservation that is requested by Client less than 7 days out from START date of reservation. Last Minute Fee will be applied to every visit in the reservation.

Early Return to Home or Late Departure from Home:
Reservations are made to plan for sitter availability for Client's reservation dates. Therefore, Clients returning home early or departing later than the reservation will be required to pay for the full reservation amount of time, regardless of whether Client returned home early or departed later than the prior reservation scheduled. No amount of money will be refunded to Client for early returns to home or late departures from home by Client after a reservation date has been agreed upon.

ADDITIONAL FEES AND POLICIES

WINTER WEATHER CANCELLATION POLICY: Alisha's Happy Paws, LLC asks that snow be cleared away from walkways, driveways, sidewalks, railings, porches, ect. Steps must be cleared of ice, and in good repair. If going to be away in ANY months fit for possible snow or icey conditions before leaving please have a plan for snow and ice to be removed to keep all of Alisha's Happy Paws sitters, walkers, and your pet(s) safe when arriving and leaving for pet care from your home.

SEVERE OR EXTREME WEATHER NON-VACATION VISITS: Policy is normally a full 48 hour notice for cancellation prior to the start of a visit's arrival window to receive a full refund. If cancellation is due to inclement weather such as icey or snowy conditions a minimum of 3 hours notice from start of arrival window will be in effect. If your place of employment has advised you to not come in to work due to winter conditions please text or call Alisha's Happy Paws 401-965-PAWS (401-965-7297).

Alisha's Happy Paws reserves the right to change time(s) of visits accordingly in order to make sure your pet is cared for but also our sitters and walkers can arrive safely. We will always keep you updated if we will not arrive in our scheduled window. Alisha's Happy Paws reserves the right to shorten a walk or outside time not only for severe winter conditions but also for extreme heat. We want to keep your pet safe and will spend the rest of the visit giving your pet(s) play and attention indoors. If Alisha's Happy Paws ever needs to cut a visit short due to inclement weather as the Client you will be updated and refunded appropriately.

SEVERE OR EXTREME WEATHER VACATION VISITS: If your vacation or time away cancellation is due to inclement weather, such as icey or snowy conditions and your vacation or time away has been postponed or delayed a minimum of 3 hours notice from start of arrival window of your first visit will be in effect. Your reservation will be held as a CREDIT on your account for your entire booking fee for when the Client is able to proceed on their trip. Cancellation winter conditions please text or call Alisha's Happy Paws 401-965-PAWS (401-965-7297) as soon as possible.

Any visits not needed due to severe or extreme weather will be held as a credit for up to six months from start date of original booking dates. If your dates will change entirely due to extreme or severe weather and Alisha's Happy Paws can not accommodate your new dates 50% of your reservation fee will be refunded to the card the Client has on file and 50% will be held as a credit for future services with Alisha's Happy Paws held up to 6 months from start date of original booking. After 6 months, the credit will no longer be of use to be put towards future visits.

When vacationing in the winter months or any month that could potentially have severe weather conditions please always have a back up in your neighborhood that could care for your pet(s) in the unfortunate event Alisha's Happy Paws sitters, walkers, or backup sitters and walkers can not safely make it to the Client's home. Client will be updated in this unfortunate event and will return to your pet as soon as safe to do so.

WEEKEND PRICING on ALL visits: Weekends include Saturday and Sunday of every week with the exception of a Holiday Weekend. Holiday Weekends will be priced as holiday and weekend pricing. Weekend Pricing: An additional $5 per visit and $10 per overnight will be assessed on all bookings that are on a weekend.

Holiday Pricing: An additional $10/visit, $10/hour for Pet Taxi, & $25/Overnight Updated 2024 Holiday Period Includes and Not Limited To: Easter Weekend (Vacation/As Needed: Friday-Monday, Reoccurring: Monday Only), Mother's Day (Sunday), Memorial Day Weekend (Vacation/As Needed: Friday-Monday, Reoccurring: Monday Only), Father's Day (Sunday), Independence Day (Vacation/As Needed: Friday-4th of July, Reoccurring: 4th of July Only), Labor Day Weekend (We will be closed this holiday weekend), Columbus Day Weekend Vacation/As Needed: Friday-Monday, Reoccurring: Monday Only), Thanksgiving (Vacation/As Needed: Thanksgiving Week (THANKSGIVING DAY WILL BE TWO TIMES HOLIDAY PRICING), Reoccurring Thanksgiving-Friday), (THANKSGIVING DAY WILL BE TWO TIMES HOLIDAY PRICING), Christmas and New Year's Day Week (Vacation/As Needed: Week of Christmas & New Year's Day, (CHRISTMAS DAY WILL BE TWO TIMES HOLIDAY PRICING) Weekly: Christmas Eve & Day, New Year's Eve & Day) (CHRISTMAS DAY WILL BE TWO TIMES HOLIDAY PRICING).

Holiday Weekends will be priced as holiday & weekend pricing. Holiday Pricing: An additional $10 per visit and $25 per overnight will be assessed on all bookings that fall within the holiday period.

PUPPY PRICING (under 1 year of age at the time of the visit), SENIOR DOGS, OR ANY DOG THAT MAY REQUIRE EXTRA OUTSIDE TIME DURING THE NIGHT: No additional pricing for puppies on visits during the day. Only overnight visits for any household with a dog that will be less than 1 year at the time of the visit, senior dogs, or any pets that need additional care during sleep hours. (An overnight visit takes place between the hours of 10pm-6am. Sleep hours include the hours of 11pm-5am.) will be an additional $25 per overnight.

Alisha's Happy Paws reserves the right to include any additional pricing for any pets that may need additional care and/or cancel if the sitter's health or safety is at risk. This includes at the time of visits and not limited to pets that may be ill, pets that show aggressive tendencies, ect.

Terms:

To request any and all visits with Alisha's Happy Paws I, the Client agree to these terms and fully understand this agreement entirely.

I have read the above and by acknowledging in the app Precise Petcare, I agree to the conditions listed above.

Alisha's Happy Paws LLC- Key(s) & Lockbox
Key(s) and Lockbox:

Alisha's Happy Paws, LLC recommends a key pad front door lock, garage code entry, and/or a lock box on the property with the key stored inside to avoid key return/pick up fees. It is not recommended to have Alisha's Happy Paws, LLC sitters hold onto keys but if that is desired by client- two keys should be provided at meet and greet allowing sitter to try both keys. Two modes of entry on the Client's premises is HIGHLY recommended in the event one fails. The Client will still be charged for the visit even if the visit was not able to be completed due to not being able to enter the Client's home. Should the Client (and emergency contact) not be responsive for a minimum of 12 hours, the Client gives Alisha's Happy Paws, LLC authority to contact a locksmith on their behalf to gain access to the home to ensure all pets do not go without food and water. Client agrees to and assumes full responsibility for the payment and/or reimbursement for any locksmith charges. Locksmith will invoice the client for these charges. If locksmith needs payment sooner Alisha's Happy Paws will pay the locksmith directly and these charges will be added to the Client's invoice and payment will be charged same day of service unless otherwise discussed.

Key Fees (if spares were not available at meet and greet): $15 Hand delivery- Pick up or return (one way). (This is the fee based on pick up/return being no more than 12 miles from Alisha's Happy Paws, LLC home base in North Smithfield. Mileage fees are as follows for Key pick ups and for visits: 13-16 miles away from Alisha's Happy Paws will be an additional $3/visit, 17-19 miles $6/visit, 20-22 miles $9/visit, and if 23+ miles away add $1 for each mile over and including mile 23/visit.

Alisha's Happy Paws, LLC does not recommend the Client hide a key outside of the house. However, should the client do this, or ask Alisha's Happy Paws, LLC or its providers to hide their key upon the end of a booking, the Client agrees to hold Alisha's Happy Paws, LLC or its providers harmless for any acts of burglary, theft, or other acts. Alisha's Happy Paws, LLC will do everything they can to be as discreet as possible, however, Alisha's Happy Paws, LLC does not recommend hiding a key outside the Client's home for any reason.

Alisha's Happy Paws, LLC does not recommend leaving Client's home unlocked in any way. However, should the client do this, or ask Alisha's Happy Paws, LLC or its providers to leave door unlocked upon the end of a booking/visit, the Client agrees to hold Alisha's Happy Paws, LLC or its providers harmless for any acts of burglary, theft, or other acts. Alisha's Happy Paws, LLC will do everything they can to be as discreet as possible, however, Alisha's Happy Paws, LLC does not recommend the Client's home be purposely left unlocked for any reason. If Client's door is left unlocked accidentally, Alisha's Happy Paws, LLC Client agrees to hold Alisha's Happy Paws, LLC harmless for any acts of burglary, theft, or other acts.

Alisha's Happy Paws, LLC will gladly provide a lockbox to be used to have access to your pets to be used by Alisha's Happy Paws, LLC and its providers ONLY free of charge. If lockbox is not able to be returned or damaged while on the Client's property Client agrees to pay a $35 fee to Alisha's Happy Paws, LLC.

Terms:

The terms of this contract apply to all pets owned by the Client, including any and all new pets that the Client obtains on or after the date that this contract is acknowledged,at any and all locations the Client designates for service.

By acknowledging in the app Precise Petcare to the terms of Alisha's Happy Paws, LLC, the Client agrees they have read this agreement in its entirety and fully understands and accepts its terms and conditions. To request any and all visits with Alisha's Happy Paws I, the Client agree to these terms and fully understand this agreement entirely.

I have read the above and by acknowledging in the app Precise Petcare, I agree to the conditions listed above.

I the Client, understand the above contract and I am in acknowledgement that I am hereby legally bound.

I am the SOLE owner of the pet(s) in care of Alisha's Happy Paws, LLC or have shared this contract with ALL owner(s) of the pet(s). ALL possible owners of the pet(s) have agreed to the above contract and are legally bound.

Alisha's Happy Paws LLC- Veterinary Release
Pet Illness/Veterinary Care/Restocking Supplies

Client is aware and agrees to the medical costs of any fees should they arise from animal(s) being ill or otherwise needing veterinary care. Client agrees to pay Alisha's Happy Paws, LLC the sum of $58 minimum and/or $58 per hour should the sitter need to take the pet(s) to vet including travel time and/or if sitter is needed to stay with the pet during veterinary care.

Client is responsible for how they see fit to protect their pets from any thing that the pet may come into contact with in their area including and not limited to vaccinations, flea and tick medication, heart worm medication, ect. Alisha's Happy Paws, LLC is not responsible or liable for anything that a pet may come into contact with while with Alisha's Happy Paws, LLC. Client takes on any risk that comes with a pet being outside at any point of a visit with Alisha's Happy Paws, LLC.

Alisha's Happy Paws, LLC along with the state of Rhode Island requires all dogs and cats over the age of 4 months be vaccinated against Rabies. The primary or first Rabies vaccination is valid for one year. One year after initial vaccination all dogs must have a rabies booster. Following the first yearly rabies vaccine, all dogs and cats must have yearly or three year boosters of the rabies vaccine. Client agrees to provide a copy of any dog or cat that will be cared for by Alisha's Happy Paws rabies certificate that provides expiration date of the vaccine from pet's veterinarian. If a veterinarian has recommended a legal exemption from your state Alisha's Happy Paws, LLC would need to see the exemption document.

If the sitter needs to restock any supplies that the Client agrees to provide but that are not provided at the time of the visit (such as pet food or medication), Client agrees to pay Alisha's Happy Paws, LLC a $58 minimum or $58 per hour restocking fee with potential mileage fee. All items restocked, client is aware and agrees to reimburse item(s) cost with Alisha's Happy Paws, LLC.

Veterinarian Release

Client's Pet will be taken to Veterinary Clinic listed in Precise Petcare, emergency vet listed in Precise Petcare, OR THE NEAREST EMERGENCY VETERINARY CLINIC WHICH CAN ASSIST THE PET(S) IN RECEIVING MEDICAL CARE AND TREATMENT.

In the event Alisha's Happy Paws, LLC and its providers is unable to contact the Client or their emergency contact directly, the Client gives permission to Alisha's Happy Paws, LLC to make medical treatment decisions (excluding euthanasia) and approve veterinary charges up to the total of $1000 (one thousand dollars) per pet.

Should Client disagree with this monetary amount, Client agrees to email Alisha's Happy Paws, LLC at happypaws_alisha@yahoo.com prior to the first date of service should they want to authorize a different amount.

Email must state, “I, (state your first and last name) authorize Alisha's Happy Paws, LLC and its pet sitters to approve veterinary charges up to $___. I assume full responsibility for the payment and /or reimbursement for any and all veterinary services up to this value.” If the email does not state this exactly it is null and void and the Client agrees to approve veterinary charges up to the total of $1,000 (one thousand dollars) per pet.

Client assumes full responsibility for the payment and/or reimbursement for any and all veterinary services rendered including, but not limited to diagnosis, treatment, medical supplies, and boarding. Client will pay veterinary clinic directly.
Client agrees to pay Alisha's Happy Paws, LLC the sum of $58 minimum and or $58 per hour for drop off to vet (one-way) and any associated fees assessed by Alisha's Happy Paws, LLC for emergency transportation, care, and/or supervision. If Client needs a sitter from Alisha's Happy Paws to stay at the veterinary clinic an hourly rate of $58 per hour will be assessed.

Terms:

The terms of this contract apply to all pets owned by the Client, including any and all new pets that the Client obtains on or after the date that this contract is acknowledged,at any and all locations the Client designates for service.

By acknowledging in the app Precise Petcare to the terms of Alisha's Happy Paws, LLC, the Client agrees they have read this agreement in its entirety and fully understands and accepts its terms and conditions. To request any and all visits with Alisha's Happy Paws I, the Client agree to these terms and fully understand this agreement entirely.

I have read the above and by acknowledging in the app Precise Petcare, I agree to the conditions listed above.

I the Client, understand the above contract and I am in acknowledgement that I am hereby legally bound.

I am the SOLE owner of the pet(s) in care of Alisha's Happy Paws, LLC or have shared this contract with ALL owner(s) of the pet(s). ALL possible owners of the pet(s) have agreed to the above contract and are legally bound.

Permission to Administer Medication
This includes and not limited to any prescribed, melatonin, CBD, injections, supplements, ear drops, topical treatment, ect.

Please list each medication with clear and concise instructions in your pet's profile in Precise Petcare including type, dosage, any special instructions (how to administer, if the medication is time sensitive- give times and/or window, if should be given with food, ect.) and possible side effects. If a start and end date are applicable please include. Even if you do not plan to have Alisha's Happy Paws, LLC administer the medication all medications your pet is on should be clearly written in their profile.

If no medications or supplements are needed for your pet(s) at this time please state that on your pet(s) profile.

Any changes, it is the Client's responsibility to inform Alisha's Happy Paws, LLC immediately by keeping your pets' profiles in Precise Petcare up to date. This includes any allergies or sensitivities. Any changes, it is the Client's responsibility to inform Alisha's Happy Paws, LLC to any and all allergies or sensitivities of any pet in the home of the Client in the software Alisha's Happy Paws uses Precise Petcare..

Any directions or information about the medications from your pet(s)' veterinarian can be uploaded into your pet(s)' profile as well.

Please read and acknowledge the following:

I release Alisha's Happy Paws, LLC and Alisha's Happy Paws, LLC sitters and/or walkers from any liability from administering any medication or supplement.

If any changes to the information in my pet(s)' profile including but not limited to medication, allergies or sensitivities, it is the Client's responsibility to inform Alisha's Happy Paws IMMEDIATELY.

I, the Client AGREE I am the SOLE OWNER(S) OF THE PET(S) that are in care of by Alisha's Happy Paws, LLC sitters or walkers and/or ALL OWNERS AGREE to not hold Alisha's Happy Paws, LLC or Alisha's Happy Paws sitters or walkers liable for any risks or side effects from administering the medication(s).


Alisha's Happy Paws- Rabies Vaccination Requirement
Any household with a dog or cat agrees to upload their pet's rabies certificate in their pet's Precise Petcare profile.

Alisha's Happy Paws, LLC along with the state of Rhode Island requires all dogs and cats over the age of four months be vaccinated against Rabies.

The primary or first rabies vaccination is valid for one year. One year after initial vaccination all dogs and cats must have a rabies booster. Following the first years rabies vaccine, all dogs and cats much have yearly or three year boosters of the rabies vaccine.

Client agrees to provide a copy of any dog or cat that will be present in the home during care by Alisha's Happy Paws, LLC by uploading it into Precise Petcare. Rabies certificate must provide the date given and expiration date of the the vaccine from the pet's veterinarian.

If a veterinarian has recommended a legal exemption from your state, Alisha's Happy Paws, LLC would need to see the exemption document. Please upload into Precise Petcare.
